After our 15-20-minute consultation I will submit your benefits information from the above insurance and will be able to verify before your initial session to determine insurance coverage and if there is any financial responsibility on your part.
If you are not sure about your coverage, please contact your insurance provider's Member Services and ask the following questions to help determine your benefits:
Does my health insurance plan include out-of-network mental health benefits?
Do I have an out-of-network deductible? If so, what is it and have I met it yet?
Does my plan limit how many sessions per calendar year I can have? If so, what is the limit? What is the start date of the calendar year my out-of-network policy is based on?
What is my co-pay (dollar amount) or co-insurance (percentage amount)?
Is a referral needed from my primary care physician?
If you do not have insurance, or do not wish to use your insurance, my out of pocket rates are as follows:
Psychotherapy, 45 minutes: $200
Psychotherapy, 60 minutes: $210
Initial Assessment: $220
I offer a small amount of sliding scale spots saved for individuals with financial need, so please inquire if needed.
